Health/Medications Disclaimer
If I do not feel comfortable disclosing information requested in the Confidential Information Form, I understand that I can request a confidential conversation with the Associate Dean for Global Learning.

I understand the information I provide will be held in confidence and will only be used by Global Learning and the College's educational partners to protect and promote student health.  I will notify Global Learning about any changes in my health status as needed.

Should I require medical care during my study away program, I understand Global Learning and the College's educational partners will offer and assist with the provision of medical care and that I will incur financial obligations for uninsured medical care.

I understand that I am expected to supply and carry my own over-the counter or prescribed medications, am responsible for prescribed health regimens, and will disclose to Global Learning and the College's educational partners any significant changes in health status during the course of study away.

I understand some medications available in the United States may be illegal or subject to border controls in other parts of the world.  I understand I must adhere to the rules and regulations of the host country, which includes, but is not limited to, the importation and use of prescription and over-the-counter medications.  It is my responsibility to verify the legality and availability of my medications prior to departure and to consult with my personal physician as necessary.  Concordia College assumes no responsibility for any consequences incurred due to insufficient research or adherence to these regulations.

As a participant in a Concordia College study away program, I understand that providing inadequate or false information about my health care coverage or health status can result in dismissal from the program.  If dismissed, I would be accountable for any additional costs related to my dismissal.
